Plan Information
The Transportation Communications International Union-Los Angeles Metropolitan Transportation Authority Health & Welfare Trust Fund provides eligible participants with the option to enroll in one of three different Medical Plans and in one of two different Dental Plans. Participants are able to enroll in either of the Medical and Dental Plans offered by the Trust Fund when they first become eligible under the Fund and also during the annual open enrollment period.
In addition the Fund also provides Vision Benefits, Prescription Drug Benefits, Life Insurance and Wellness Benefits.
Medical Plans
TCU-LAMTA Health & Welfare Fund Fee-For-Service Medical Plan: If you enroll in this Medical Plan you and your enrolled dependents will be covered under the Trust's Fee-For-Service Medical Plan for hospital, medical services and supplies. You may use any Physician or Hospital in the United States. If you use a Preferred Provider Organization (PPO) Hospital, from the network that the Fund has contracted with, you will reduce your co-payments and out of pocket expenses. Consult your Summary Plan Description for further information on the Fund's PPO network. You may request a listing of the PPO network from the Administrative Office. A schedule of benefits is shown in the Funds Summary Plan Description, which can be viewed on this web site or requested, from the Administrative Office.

Kaiser Medical Plan: If you choose the Prepaid Medical Plan provided by Kaiser Permanente you and your enrolled dependents must receive all medical care through Kaiser offices and hospitals. Services that are prescribed or directed by a Kaiser physician are provided either at no charge to you or at specified co-payments. You must live within the service area of any Kaiser medical facility in order to enroll in this plan. Kaiser will provide you with a booklet describing the services and benefits. You may request a booklet from the Administrative Office.

United Healthcare Medical Plan: If you choose the Prepaid Medical Plan provided by United Healthcare you and your enrolled dependents must receive all medical care through the participating medical group or physician you have selected within the United Healthcare group of providers. Services that are authorized by your United Healthcare physician are provided at no charge or at specified co-payments. You must live within the service area of the facility you will be using in order to enroll in this Plan. United Healthcare will provide you with a listing of the facilities as well as a booklet describing the services and benefits. You may request a booklet from the Administrative Office.

Sav-Rx Prescription Drug Program
Sav-Rx Prescription Drug Program: Eligible participants enrolled in the TCU-LAMTA Health & Welfare Fund Fee-For-Service Medical Plan must obtain prescription drugs at Sav-Rx participating pharmacies. Using the Sav-Rx pharmacies will reduce participants cost and the Funds prescription expenses. Participants are required to show their Sav-Rx Card at the Sav-Rx pharmacy when filling a prescription. Participants then submit their prescription drug claim to the Fee-For-Service Medical Plan for reimbursement. Sav-Rx participating pharmacies can be located by using the Sav-Rx link found on this web site.
Dental Plans
TCU-LAMTA Health & Welfare Fund Fee-For-Service Dental Plan: If you enroll in this Dental Plan you and your enrolled dependents will be covered under the Trust's Fee-For-Service Dental Plan you may use any licensed Dentist in the United States. After meeting the applicable deductible, the Trust will pay a portion of the covered dental charges. The schedule of benefits is shown in the Funds Summary Plan Description, which may be viewed on this web site or requested, from the Administrative Office.
First Dental Health Preferred Provider & Exclusive Provider Dental Network: Eligible participants enrolled in the TCU-LAMTA Health & Welfare Fund Fee-For-Service Dental Plan have the option to utilize both the First Dental Health Preferred Provider (PPO) & Exclusive Provider (EPO) Dental Networks. Using a First Dental Health Network Dentist will reduce dental costs for you and also for the Fund.
United Concordia Dental Plan: If you choose to enroll in the prepaid Dental Plan provided by United Concordia you and your enrolled dependents will be covered under the United Concordia Prepaid Dental Plan and must receive all dental care through your selected United Concordia Dentist. Dental services that are authorized by your United Concordia Dentist are provided either at no charge to you or at specified co-payments. United Concordia will provide you with a booklet describing the services and benefits. You my also request a booklet from the Administrative Office.
EAP Benefits
An Employee Assistance Program is provided by Anthem EAP and is available to Employees, Retirees, Dependents and those in the same household of the person enrolled in the Funds health plans. Further information can be accessed by using the following links:

VSP Vision Plan: Eligible participants may receive vision coverage from VSP. VSP has an extensive network of doctors through the United States. Additional information regarding how to locate a VSP provider can be found by utilizing the VSP link provide on this web site.
